Is there a fee for Qualified Providers and Community Partners to participate?
Yes. This fee is utilized to promote, rent the equipment and site.
How do Qualified Providers and Community Partners register?
Complete the registration forms and mail in your Registration form, Insurance Certificate and Payment. Once we receive your registration form we will contact you with a confirmation.
Will Providers and Community Partners be listed on the Web Site?
Yes, once your registration is accepted.

Last Minute Vendors
Last day to submit your registration is postmarked by 5pm 10/31/11, if you cannot meet the requirements by that date, we cannot accept you at the fair.
Committee Members:
This event is put on by DDD Provider Agencies just like yours. If you are interested in being part of this committee and assist with this event or future events please complete the Contact Us tab. We will contact you and with meeting dates and times. The Committee needs new members to plan and hold the fair next year and the years to come.
